David Douglas PA-C offers focused primary care virtual phone visits.
Step one, to schedule an appointment click the “book an appointment button…” below and add yourself to the online scheduling calendar. Once completed, you will receive a confirmation text on the cell phone number you provide, confirming the appointment time you wish to be called.
After scheduling an appointment, please complete the last two steps below, as soon as possible, (prior to your visit.)
Step two, please complete the medical forms section, as soon as possible after scheduling a visit. These forms can be accessed from the main page of the Riverton Medical Website. Please submit all four forms.
Step three, please access the payment page prior to the visit and choose the appropriate payment price. You may pay by debit or credit card. Initial Visits are typically $199, and Follow Up/Virtual Visits are typically $99.
